Job Overview:

Responsible for planning, managing, implementing, and evaluating the operations of the Department of Case Management which includes oversight of effective resources utilization and the provision of high quality and excellent care. Work in collaboration with administration and physician leadership to establish measurable standards and outcomes. Oversee nurse case management and social work services. Ensure continued development of the program to improve organization performance to enhance provision of services.

Qualifications:

Required

  • Master s Degree in Social Work or related field required
  • Three years of clinical experience required.

Preferred:

  • Advanced training in palliative care preferred.

Certifications and Licenses Required:

  • NJ Clinical Social Work License (LCSW) required or obtained within 6 months of start date. If not starting with LCSW, will need LSW.

Scheduling Requirements:

  • Day position
  • FT

Essential Functions:

  • Supervises Social Work operations of the inpatient medical and surgical units.
  • Provides support and education to social work staff, including oversight of CEU programs and training. Offers clinical supervision as needed.
  • Discusses illness trajectory and advanced directives with chronically-ill and/or medically complex patients.
  • Identifies patients appropriate for palliative and/or hospice care and discusses these services/benefits with patient and family. Makes referrals as needed.
  • Exhibits strong knowledge of advanced directives (ie POLST) and laws around healthcare decision-making.
  • Identifies need for interdisciplinary or family meetings and facilitates these meetings.
  • Collaborates with department leadership to streamline processes and implement strategies to create more efficient delivery of care.
  • Ensures compliance with regulatory documentation standards.
  • Understands financial responsibilities of the case management department and safeguards system resources by actively participating in utilization management and insurance communication. Collaborates closely with physician advisor.
  • Participates in interdisciplinary rounds and facilitate communication with the interdisciplinary care team.
  • Completes psychosocial assessments and implement patient-centered care plans that meet needs identified in assessments.
  • Discharge planning activities: seamless and timely coordination of care including: referrals to home care, DME, SNF/ rehab placements, hospice, community resources, and transportation arrangements.
  • Participates in quality data collection and risk management referrals
  • Collaborates with department leadership regarding ongoing performance management of front-line social workers.
  • Ensures that staff operate within compliance with all applicable policies, laws and regulations.
  • Participates in quality data collection
